Liability Waiver

1. Nature of Coaching Services

I understand that the health, wellness, and lifestyle coaching services provided by Aevum Wellness are intended for general wellbeing, personal development, and goal setting. These sessions are not a substitute for professional medical advice, psychological therapy, psychiatric care, or licensed therapeutic treatment. I understand that coaching is a collaborative process and not a prescribed treatment.

 

2. No Guaranteed Outcomes

I acknowledge that coaching results are individual and vary from person to person. Aevum Wellness does not guarantee any specific outcomes, diagnoses, cures, or medical improvements as a result of the coaching process.

 

3. Assumption of Risk & Release of Liability

I recognize that my personal development requires my active participation and honesty. I voluntarily agree to participate in these coaching sessions and accept full responsibility for my own physical, mental, and emotional wellbeing and any resulting choices or actions.

 

I hereby release, waive, and discharge Aevum Wellness, its coaches, and agents from any and all liability, claims, demands, or causes of action that I may now or in the future have for any loss, injury, damage, or expense (including legal fees) arising from or in connection with the coaching services, whether offered free of charge or for a fee.

 

4. Client Responsibility

I agree that it is my responsibility to:

Seek professional medical advice, diagnosis, or treatment for any physical or mental health concerns before and during coaching.

Inform my healthcare providers of any lifestyle or wellness changes I undertake.

Not rely on coaching as a sole source of advice for medical, psychological, legal, or financial matters.

 

5. Confidentiality

Aevum Wellness will keep all information shared during coaching sessions strictly confidential. Information will not be disclosed to any third party without my written consent, except where required by law (e.g., if there is a reasonable suspicion of risk of harm to myself or others, or in cases of suspected abuse).

 

6. No Professional Licensing

I understand that the coaches at Aevum Wellness are not licensed physicians, psychologists, therapists, or dietitians unless explicitly stated otherwise. The services provided are coaching and not regulated healthcare.

 

7. Mutual Right to Discontinue Service

I confirm that I am participating in these sessions voluntarily and without coercion. I understand that I may discontinue coaching at any time. I acknowledge that Aevum Wellness likewise reserves the right to discontinue service at its discretion, including but not limited to reasons such as inappropriate client behavior, non-payment, or a conflict of interest.

 

8. Payment and Refund Policy (If Applicable)

For paid sessions, I agree to the fee structure, payment terms, and refund policy as outlined on the Aevum Wellness website.

 

By agreeing to this waiver, I confirm that I have read, understood, and voluntarily agree to all the terms outlined above. I am competent and of sound mind to enter into this agreement.
